Sage Integrative Psychiatry is not contracted with any insurance companies. However, we provide Superbills after visits so that you can receive reimbursement via out-of-network benefits from your insurance company. This comes in the form of a PDF attachment via e-mail and contains all the information you will need to submit to your insurance company.

Pre-Appointment Information
Prior to your appointment, we will be sending you an email that will include the forms you will need to fill out electronically prior to your visit.
